Please read these instructions carefully.
1) Do not eat or drink anything for at least 4 to 6 hours before the procedure. This includes your regular medication. 2) You should arrange that you be taken and fetched by someone as you will not be able to drive a vehicle yourself after the procedure. 3) Do not take any alcohol before the procedure and not for 6 hours after the procedure. 4) To get authorisation from your health insurance one needs to submit to your medical aid details including 1) diagnosis code and 2) relevant procedure code(s). This information needs to be linked to the Practise Number 0490989 (New Street Sedations). The relevant codes required will be provided by your doctor doing the procedure. Your medical aid will then give you an authorisation number to bring with you on the day of your planned procedure. 5) Your doctor may recommend Pre –Operative medication to prepare you for your procedure called Dormicum (see below). It is however not always necessary. 6) If you are a child you would benefit from using an Emla patch as local anaesthetic preparation for drip insertion (see below). Your doctor will advise you if you need to prepare for this. 7) If possible download and complete the adjacent English or Afrikaans instruction forms and bring them with you. They will also be available when you arrive. Dormicum (7.5mg). This tablet if prescribed is very important and should be taken with some water half an hour before the time of the procedure. This tablet can make you drowsy and might result in a loss of memory of the procedure. Dose : Children and Adults 8 yrs and older take a full tablet : Children younger than 8 yrs should take 1/2 tablet (you may crush it) The EMLA patch: This patch will only be given to children. Using this patch on the skin will numb the skin so that an injection can be given to your child without any pain Please place this patch on the skin after cleaning the skin well with the supplied alcohol swab. Look for the biggest and thickest vein on the top of the hand or upper part of the arm and leave it in place for at least 2 hours before the operation. If you are uncertain about the placement of the patch, ask your Dentist or Doctor. Payments: The practice will submit your account on your behalf to your medical aid, but still keep you responsible for payment in the event that your medical aid should, for whatever reason, fail to settle the amount in full.
